Graduate Student Forms
Most forms for graduate students are required by the University of Minnesota Graduate School, and are available from the Graduate School forms website.
There are also several departmental forms specific to the SJMC that graduate students often request. The most commonly requested departmental forms are listed below. These forms are in PDF format, which require the Adobe Acrobat reader to view. Download the Adobe Acrobat reader.

Departmental M.A. Degree Program Form (PDF)
M.A. students should submit this form to the Graduate School at least two semesters before the term in which they plan to take the preliminary oral examination.

Ph.D. students should submit this form to the Graduate School at least two semesters before the term in which they plan to take the preliminary oral examination.
Application for Kriss research funds (PDF)
Kriss Research funds are designed to help support SJMC graduate students who are pursuing non-course-related research.
Application for Casey Dissertation Research Award (PDF)
Instructions and information regarding the Casey Dissertation Research Award. To apply for the award, students should use the Graduate School's Doctoral Dissertation Fellowship application.
