Activity Report Form

• The Student Activity Report Form is due each year on February 1. This form must be completed by each student, regardless of financial support status.

Thesis (Dissertation) Proposal Form

• Thesis Proposal Form (PDF)
  Before undertaking dissertation research, Ph.D. students, in consultation with their advisers, must prepare a written proposal, fill out a SJMC Thesis Proposal Form and hold a thesis proposal meeting with the dissertation committee members. The adviser and all committee members must sign the Thesis Proposal Form if they approve the student's proposal. Then, the student must submit the signed form to the SJMC Student Services Office, 110 Murphy Hall. For more information, click here.

Travel Authorization

Please submit all paperwork to Angie Rehn in 110 Murphy Hall.

• Conference Pre-Approval Form
  Submit at least 10 working days before you submit your paper to a conference. The pre-approval process is to encourage thoughtful submission of your work.
• List of Recommended Conferences
  Students do not need adviser permission to attend these conferences.
• Travel Authorization Form
  Submit at least 5 working days before travel begins. Authorization should be obtained before any trip charges are incurred.
• Graduate Travel Expense Worksheet
  Submit immediately after returning from your conference travel along with receipts for expenses you have occurred.
• Employee Expense Worksheet with directions
  Submit within two weeks after returning from travel.
